By submitting this form you are acknowledging that you agree to all policies as follows:

Any deposit paid is non-refundable under any circumstance.
If you no-show your appointment, your deposit is forfeit.
If you reschedule or cancel less than 48 hours before your appointment, your deposit is forfeit.
If you reschedule more than twice, your deposit is forfeit.
If you cancel without arranging a new appointment date, after six months, your deposit is forfeit.
If your deposit is forfeit for any of the above reasons, you will need to pay a new deposit to be booked in and your previous deposit will not be counted toward your service.
